(Accommodation support in your own home or in a shared living arrangement).

Under the funding guidelines of the NDIS and the National Disability Services Standards, we will assist you to live as independently as possible in your own home. Our focus is: 

• To ensure that every minute of support counts and you are working towards goals identified.
• To ensure you and or family are involved in the selection of your support team.
• To provide the necessary supports in areas of daily living such as personal care, domestic assistance, meal preparation and skill development.
• To assist you to identify and establish your goals and provide the support team with your expectations of delivering the support. 

With the use of our Client management System, you, your family and approved representative will be able to access your documentation that identifies progress made towards goals as well as challenges and risk mitigation processes.   

With the Directors' extensive experience in this area, our role is to assist our clients in implementing the goals identified in their NDIS plan. This role will also include: 

• A component of managing points of crisis;
• Assisting to prepare the report for the NDIS review every 12 months demonstrating evidence of achievements and challenges;
• Commit to working with you closely in ensuring you have opportunities to be connected to your community with the right supports, assist you to develop capacity to managing your own supports into the future and understanding your needs. 

To assist you to successfully engage in your community with a focus on: 

• Building new interests, 
• Developing friendship groups, 
• Building socially valued roles, 
• Achieving your goals identified in your NDIS plan, 
• Aiming to reduce the need of more intensive longer term support needs,
• Building inclusive communities and; 
• Improving your general health and wellbeing.
